Established Patient Visit

For current patients (seen within 2 years).

CDL / DOT Medical Exam $98.00

Select this option if you need a Medical Exam for your Commercial Driver's License.

Acupuncture

Select for 30 minute acupuncture appointment. Must be a current patient.

Drug Testing

Select if you need urine or hair drug testing.

DNA Paternity Testing

Paternity testing requires samples from all three parties; however, the samples do not need to be obtained at the same visit.
